Short Description of Organisation 

Bulwell and Top Valley Primary Care Network (PCN) brings together local GP practices and community organisations to make it easier for people to get the right care and support when they need it.

Formed in 2019, we work together to provide joined-up, personalised care for our community, supporting around 47,800 patients across five practices:

Leen View Surgery, Parkside Medical Practice, Southglade Medical Practice, The Practice St Albans, and Rise Park Surgery.

We’ll be joined by local organisations, as well as our social prescribing team, who can offer expert advice and connect you to local support and activities to help you stay healthy this winter.
